आज, तपेदिक या Tuberculosis, अभी भी दुनिया के सबसे बड़े संक्रामक हत्यारों में से एक है, इसमे मलेरिया या एचआईवी और एड्स और अन्य बिमारीयो से अधिक मौतें होती हैं। लेकिन वास्तव में यह बीमारी क्या है, और यह रोगजनक इतने लंबे समय तक कैसे बना रहता है? आइये जानने की कोशिश करते है
Tuberculosis hindi meaning टीबी क्या है
(टीबी)एक संक्रामक रोग है जो माइकोबैक्टीरियम टुयुबरक्लोसिस नामक जीवाणु,के कारण होता है। ये जीवाणु मरीज के किसी भी भाग को प्रभावित कर सकता है,परंतु ज्यादातर यह फेफडो पर प्रभाव डालता है इसे पल्मोनरी टीबी कहा जाता है, फ़ेफडो के आलावा ये शरीर के किसी भी भाग को प्रभावित कर सकता है इसे एस्ट्रापल्मोनारी टीबी कहा जाता है, पल्मोनरी टीबी से,पीड़ित व्यक्ति के द्वारा हवा के माध्यम से इस बिमारी को एक इंसान से दुसरे इंसान तक आसानी से फैला सकता है। प्रत्येक संक्रामक रोगी एक वर्ष में 10 -15 व्यक्तियों को संक्रमित कर सकता है जब तक कि प्रभावी ढंग से इलाज न किया जाए। टीबी को और भी कई नामो से जाना जाता है जैसे क्षय रोग, राजरोग, तपेदिक, क्ष्मारोग, माहामारी, इत्यादि
Tuberculosis mode of Transmission
दरअसल टीबी बैक्टीरिया से होनेवाली बीमारी है,जो हवा के माध्यम से एक व्यक्ति से दूसरे व्यक्ति में फैलती है। यह सामान्यतः फेफड़ों से शुरू होती है। सबसे ज्यादा फेफड़ों की टीबी होती है, लेकिन यह ब्रेन, यूटरस, मुंह, लिवर, किडनी, गला, हड्डी जैसे शरीर के किसी भी अंगों में हो सकती है।
चूँकि टीबी का बैक्टीरिया हवा के जरिए फैलता है इसलिए खांसने और छींकने के दौरान मुंह-नाक से निकलने वालीं बारीक बूंदों से यह संक्रमण फैलता है। यदि मरीज नहीं भी खांसे तो भी उसके बहुत ज्यादा करीब बैठने या लेटने से भी इसके इन्फेक्शन होने का खतरा बना रहता है। हालांकि, फेफड़ों के अलावा बाकी टीबी एक इंसान से दूसरे इंसान में फैलनेवाली नहीं होती है और जैसा कि इसे लोग जेनेटिक कहते पर यह एक पीढ़ी से दूसरी पीढ़ी में भी नहीं होती है।
टीबी बैक्टीरिया हमारे वायुमार्ग के रास्ते हमारे फेफड़ों को संक्रमित करते हैं। इधर, मैक्रोफेज नामक प्रतिरक्षा कोशिकाएं संक्रमण स्थल पर पहुंच जाती हैं, बैक्टीरिया के आक्रमणकारियों को अवशोषित करने और तोड़ने का प्रयास करती हैं। कई मामलों में, यह प्रतिक्रिया बैक्टीरिया को हटाने के लिए पर्याप्त है।
लेकिन जिनका Immune system होता है जैसे कुपोषण और एचआईवी से लेकर मधुमेह और गर्भावस्था तक- प्रतिरक्षा प्रतिक्रिया इतनी मजबूत नहीं होती है कि टीबी बैक्टीरिया को नष्ट कर सके। उसके बाद तो माइकोबैक्टीरियम तपेदिक उन मैक्रोफेज के अंदर पुन: प्रवेश करेगा, और आसपास के फेफड़े के ऊतकों में कॉलोनियों का निर्माण करेगा। चूंकि वे अधिक कोशिकाओं को संक्रमित करते हैं, बैक्टीरिया सेल-डीग्रेडिंग एंजाइमों को नियुक्त करते हैं जो संक्रमित ऊतक को नष्ट करते हैं, और सीने के दर्द को ट्रिगर करते हैं, इसके आलावा रोगियों को बलगम मे खून आने कि सम्भवना भी बनी रहती हैं। फेफड़ों को नुकसान होने से ऑक्सीजन की कमी हो जाती है।
इससे हार्मोनल परिवर्तनों की बाढ़ शुरू हो जाते है – जिसमें भूख और लोहे के उत्पादन में कमी होने लगती है। यहां से, रोगाणु कंकाल प्रणाली में फैल सकते हैं, जिससे पीठ दर्द और कठिनाई बढ़ सकती है; गुर्दे और आंतों में, पेट दर्द का कारण बन जाती है और मस्तिष्क के लिए, सिरदर्द और यहां तक कि बिगड़ा हुआ चेतना पैदा करता है। ये लक्षण टीबी होने का संकेत देते है इसके आलवा लगातार वजन कम होना, खूनी खांसी, और अशीन त्वचा जैसे लक्षण भी पाये जाते है
